Initiates the extrinsic pathway of blood coagulation. Serine protease that circulates in the blood in a zymogen form. Factor VII is converted to factor VIIa by factor Xa, factor XIIa, factor IXa, or thrombin by minor proteolysis. In the presence of tissue factor and calcium ions, factor VIIa then converts factor X to factor Xa by limited proteolysis. Factor VIIa will also convert factor IX to factor IXa in the presence of tissue factor and calcium. Function: Initiates the extrinsic pathway of blood coagulation. Post-translational modifications: The vitamin K-dependent, enzymatic carboxylation of some glutamate residues allows the modified protein to bind calcium. The iron and 2-oxoglutarate dependent 3-hydroxylation of aspartate and asparagine is (R) stereospecific within EGF domains. DISEASE: Defects in F7 are the cause of factor VII deficiency (FA7D) [MIM:227500]. A hemorrhagic disease with variable presentation. The clinical picture can be very severe, with the early occurrence of intracerebral hemorrhages or repeated hemarthroses, or, in contrast, moderate with cutaneous-mucosal hemorrhages (epistaxis, menorrhagia) or hemorrhages provoked by a surgical intervention. Finally, numerous subjects are completely asymptomatic despite very low factor VII levels. Similarity: Belongs to the peptidase S1 family. Contains 2 EGF-like domains. SWISS: P08709 Gene ID: 2155 Database links: Entrez Gene: 2155 Human Omim: 613878 Human SwissProt: P08709 Human Unigene: 36989 Human 凝血因子7是肝臟合成的一種維生素K依賴性的單鏈糖蛋白,由4 0 6個(gè)氨基酸殘基組成,分子量約45kD。 凝血因子Ⅶ的半衰期最短(4~6h),血漿含量較低(0.5~2mg/L),故可作為肝病患者蛋白質(zhì)合成功能減退的早期診斷指標(biāo)。在慢性肝病患者通過肝活檢組織原位雜交的方法檢測(cè)到凝血因子Ⅶ的表達(dá)與肝纖維化的分級(jí)呈負(fù)相關(guān),可作為預(yù)測(cè)纖維化程度的指標(biāo)。凝血因子Ⅶ活性還與預(yù)后有著密切的聯(lián)系,經(jīng)研究認(rèn)為:凝血因子Ⅶ活性<34%的肝硬化患者93%在隨訪10月內(nèi)死亡,故認(rèn)為它是肝硬化患者預(yù)后好壞的早期預(yù)測(cè)指標(biāo),可更好識(shí)別肝移植候選人。肝硬化患者凝血因子Ⅶ活性可明顯下降,凝血因子Ⅶ缺乏可導(dǎo)致血小板活性的改變,結(jié)合血小板計(jì)數(shù)減少使出血時(shí)間延長(zhǎng),因此對(duì)有創(chuàng)診斷與治療的肝硬化患者,還應(yīng)該用凝血因子Ⅶ活性進(jìn)行出血危險(xiǎn)度的評(píng)估,而不能僅看血小板計(jì)數(shù)。除診斷之外,重組凝血因子Ⅶ可以有效地糾正肝病患者凝血異常,有利于有創(chuàng)性檢查的進(jìn)行。 |
